Activity Goal

This educational activity is intended to review current guidelines for the use of opioid analgesics in patients with chronic noncancer pain.

Intended Audience

Pain specialists, anesthesiologists, physiatrists, neurologists, palliative care specialists, primary care physicians, nurse practitioners, pharmacists, allied health professionals, fellows, and residents.

Needs Assessment and Learner's Gap

Opioids are an important analgesic option for many patients. However, careful patient selection is critical to ensure that patients are managed as safely and effectively as possible. In 2009, the American Pain Society (APS) and American Academy of Pain Medicine (AAPM) jointly issued evidence-based guidelines on the use of opioids to treat patients with chronic noncancer pain.1 Developed by an interdisciplinary panel, these guidelines address key clinical, psychological, and legal issues that may occur. Before initiating opioid pharmacotherapy, clinicians should conduct a careful history, physical examination, and assessment of risk for misuse, abuse, or diversion. Opioid analgesic regimens should be individualized based in part on the initial evaluation, and on patient-specific therapeutic goals. Clinicians should counsel patients on the risks and benefits of opioid analgesics and monitor for progress towards therapeutic goals, treatment-related adverse events, and signs of inappropriate medication use. Finally, clinicians should become familiar with local, state, and federal laws, regulatory guidelines, and policy statements relevant to opioid prescribing.
